Healthcare in the TrenchesTaking the journey along with the author we learn just how difficult it is to become a surgeon. His educational background is extensive and described in detail plus the struggles he had having to deal with the long hours he had to put in as an intern. Plus, the reporters and critics voicing opinions about the workload of an intern without really understanding their dedication and commitment to the profession. This is the core of the first half of the book before we focus on the politics of the medical profession and healthcare.Medical care has gone through many transitions and doctors face numerous obstacles when dealing with our healthcare system. Doctors want to take care of their patients but with the need for increased volume the time limits for visits have decreased . The first section of this book focuses on Dr. Badia ‘s journey to becoming a well know surgeon and physician. The second he analyzes the increasing role of the government in healthcare. The government has great influence on healthcare but the focus he states should be to find and provide help for those in financial need and less fortunate. He also focuses on areas we hear about in the news, abuses in our present system. Finally, in section 3 he creates a general overview of the problems and areas that need work but defining a plan is not a one group situation. Doctors need to work as teams, share ideas and focus on costs. Believe it or not he explains the German healthcare system a viable model .The book is well documented and researched and we are reminded of the fact that the present pandemic plays a negative role as patient co-pays have increased, healthcare costs rise, loss of health insurance and both the public and doctors have high cost-payments and deductibles. From firsthand experience I learned and must agree that patients must often voice loud opinions, fight for their rights and pray for appointments during this difficult time.Part two is where the meat of what he is explaining focuses on: Government Interference and Politics in Medicine. The role of government in medical care is hot a topic. They are or should be concerned with the welfare and defense of the people in our country and the enforcement of its laws are applicable unless the author states you are a true libertarian anarchist. The author always told the government officials that hand surgery, should be well developed and supported because the majority of industrial industries naturally involved the head, and that person can only go back to work and being productive if the sustained trauma is appropriately managed. Cost of cause comes into everything and there is a huge cost of not providing appropriate and timely hand care. He includes the HIPAA Law and explains how it came to be and why we need to sign off on it before any information to any provider or family member is given. Pages 193-194 are very explicit and the information about the HIPAA LAW is invaluable. Other areas explained are The Joint Commission for Hospital Accreditation, The Hospital System Behemoths and Bureaucracy. He begins with the gravity of the situation we are in namely the COVID19 epidemic that has brought that concert into our person purview ad understanding that hospitals are for sick people and when doctors refer to the SICK we generally mean patients who are unstable and often have chronic comorbidities that obligate them to seek urgent hospital care time and time again. The contrast this with patients who break a wrist or develops a severe sore throat with fever. The author goes on to explain the gravity of many illness and the fact that COVID19 and other pandemics clarify the distinction between the sick and those who require a hospital visit and admission and those than can undergo conservative treatment on an outpatient basis. Chapter 8 elaborates more in this area and includes hospital negligence and follows in Chapter 9 with The Big Business of Insurance and that is one topic that affects everyone. The frustrations we face with dealing with healthcare, the case studies he presents and the stories from fellow doctors who share his and our frustrations are shared in this well researched book.The Big Business of Health Insurance is a major concern of everyone and an unavoidable component of life which everyone must deal with at some time in life. You pay an affordable, recurring fee on a regular basis in exchange for the security you hope will come along with it when something unforeseen happens. Should be easy but it is not. The insurance company takes the risk and for that fact, they make the money, somewhere along the way, this transaction has become distorted.The remainder of the chapter focuses on pros, cons and different experiences that are cited by subscribers. All in the author included a serious question for all of us to consider: DO Health Insurers or Doctors know Best? A well-known surgeon talks about this in detail: Read what Surgeon :A.J. DiGiovanni recounts: A real case with serious consequences. Followed by Workers’ Compensation and Kerry Anderson’s story.Chapter 10 is vital for the reader: Opportunities Abound Cashing in on the System. The chapter that is most revealing is the one that focuses on Medical Malpractice, Trial Lawyers and Tort Reform. “No discussion of skyrocketing healthcare costs is complete without a thorough examination of: Medical malpractice, trial lawyers and tort reform.” The author reminds us of the infamous McDonald’s coffee case from 1992. Different cases are sited, and the reader will become enlightened when you read Dr. Mark Rekant on Malpractice Insurance Costs and other Hidden Healthcare costs. This book is well researched and there are many other areas discussed such as Moral Injury and the Future of Medicine, Doctors and Money: The Great Divide and the most important question: Where do you go from here?
